Scope: 

Version 1.0 of this application will provide a self service component for interfacing with My ASU so that a new incoming graduate student can identify address to mail their I-20 (F1 visas) or DS-2019 (J1 visas). Specifically, this functionality will provide the following:
* Select Home or Mailing address, or input a new address for mailing of I-20 or DS-2019
* Identify mailing option
* Identify alternate person to pick up form at Graduate College on behalf of student
* Identify telephone number for recipient if form will be picked up

Once form is submitted, student may access again to view status of form and if shipped, view tracking number for Federal Express or other shipper.

On the back end, the AD0068 mod will be modified to track submissions of forms, view credit card transaction status and enter FedEx tracking numbers. A Nelnet reconciliation process will be integrated in with AD0089.
